Оптимизация терапевтических подходов к лечению синдрома абдоминальной боли у детей с синдромом перекреста билиарная дискинезия + синдром раздраженного кишечника.</p><p>Характеристика детей и методы исследования. В исследование были включены 309 детей в возрасте от 13 до 17 лет. Синдром раздраженного кишечника был диагностирован у 76 (24,6%) пациентов в соответствии с Римскими критериями IV пересмотра и результатами лабораторно-инструментального исследования. У 45 (59,2%) пациентов отмечался вариант синдрома с запором, у 19 (25%) – вариант с диареей, у 12 (15,8%) – смешанный вариант (чередование запора с диареей). Билиарная боль выявлена у 58 (76,3%) пациентов, в том числе у 34 (44,7%) пациентов с запорами, у 13 (17,1%) с диареей и у 11 (14,5%) со смешанным вариантом. В лечении использованы два варианта терапии: препарат тримебутин (Тримедат, «Валента Фарм», Россия) в возрастной дозировке в течение 28 дней (основная группа) и папаверин в возрастной дозировке также в течение 4 нед (группа сравнения).</p></sec><sec><title>Результаты</title><p>Результаты. В основной группе наблюдалось более выраженное снижение интенсивности боли в животе, ассоциированной как с синдромом раздраженного кишечника, так с билиарной дискинезией. Различия между результатами лечения в основной и контрольной группе статистически значимы (p&lt;0,01). В период лечения в основной группе ни одного случая развития побочных эффектов не зарегистрировано, различия по частоте побочных эффектов между основной группой и группой сравнения статистически значимы (p=0,01).</p></sec><sec><title>Заключение</title><p>Заключение. Коррекция абдоминальной боли тримебутином значительно превосходит по эффективности стандартную спазмолитическую терапию при лечении overlap-синдрома синдром раздраженного кишечника + билиарная дискинезия. Терапия тримебутином безопасна и может быть рекомендована в качестве достаточно эффективного метода лечения детей с overlap-синдромом.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. To optimize therapeutic approaches to the treatment of abdominal pain syndrome in children with overlapping syndrome of biliary dyskinesia + irritable bowel syndrome.</p><p>Characteristics of children and research methods. The study included 309 children aged from 13 to 17 years. Irritable bowel syndrome was diagnosed in 76 (24.6%) patients in accordance with the Rome IV revision criteria and the results of laboratory and instrumental studies. 45 (59.2%) patients had a variant of the syndrome with constipation, 19 (25%) – a variant with diarrhea, 12 (15.8%) – a mixed variant (alternating constipation with diarrhea). Biliary pain was detected in 58 (76.3%) patients, including 34 (44.7%) patients with constipation, 13 (17.1%) with diarrhea variants and 11 (14.5%) with mixed option. The researchers used two treatment options: trimebutin (Trimedat, ValentaPharm, Russia) in an age dosage for 28 days (main group) and papaverine in an age dosage for 4 weeks (comparison group).</p></sec><sec><title>Results</title><p>Results. The main group demonstrated a more pronounced decrease in the intensity of abdominal pain associated with both irritable bowel syndrome and biliary dyskinesia. Differences between treatment results in the main and control groups are statistically significant (p&lt;0.01). During the treatment period there were no cases of side effects in the main group, the differences in the frequency of side effects between the main group and the comparison group are statistically significant. (p=0.01).</p></sec><sec><title>Conclusion</title><p>Conclusion. Correction of abdominal pain with trimebutin significantly exceeds the effectiveness of standard antispasmodic therapy in the treatment of overlap syndrome of irritable bowel syndrome + biliary dyskinesia.
