Ни одно из используемых в настоящее время лекарственных средств для лечения судорог у новорожденных не имеет достаточной доказательной базы, чтобы можно было бы однозначно судить о его эффективности. Отсутствуют результаты крупных исследований, позволяющих принять решение о выборе противосудорожного препарата для стартовой терапии и альтернативы в случае неэффективности препарата выбора. В обзоре обобщены данные об эффективности противосудорожных препаратов различных фармакологических групп, рассмотрены их преимущества, недостатки и возможные механизмы вариабельности эффективности при применении у новорожденных. Проанализированы подходы к оценке длительности применения противосудорожных препаратов. Обсуждены проблемы и перспективы клинических исследований с целью повышения эффективности и безопасности противосудорожной терапии у новорожденных.</p></abstract><trans-abstract xml:lang="en"><p>Proper treatment of neonatal seizures is crucial for reducing long-term neurological disorders. None of the currently used drugs to treat neonatal seizures has sufficient evidentiary base to make an unambiguous conclusion about its effectiveness. There are no results of large-scale studies that allow choosing an anticonvulsant drug for starting therapy and an alternative to it, in case the drug of choice is ineffective. The review summarizes the data on the effectiveness of anticonvulsants of different pharmacologic classes and considers their benefits, drawbacks, and possible mechanisms of variability in effectiveness when used in newborns.
