Изучить исходы госпитального этапа у новорожденных в зависимости от результатов первичной оценки их состояния по шкале КШОНН (клиническая шкала оценки недоношенного новорожденного).</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Когортное исследование включало данные 604 выездов транспортной бригады реанимационно-консультативного центра к новорожденным детям, госпитализированным в медицинские организации региона и находившимся на дистанционном наблюдении в период с 1 августа 2017 г. по 31 декабря 2018 г. Медиана массы тела при рождении составила 2515 [1600; 3275] г, медиана гестационного возраста 36 [32; 38] нед. Выполнено разделение общей выборки на подгруппы в зависимости от оценки по исследуемой шкале с последующим сравнением характеристик пациентов и исходов госпитального этапа в подгруппах.</p></sec><sec><title>Результаты</title><p>Результаты. В сравниваемых подгруппах наблюдались достоверные различия по массе тела и гестационному возрасту, по мере увеличения оценки по КШОНН определялись достоверный рост доли пациентов с массой тела менее 1000 г и снижение доли пациентов с массой тела 2500–3499 г. Наибольшая доля экстремально недоношенных новорожденных наблюдалась в подгруппе 6–8 и 9–14 баллов, 30,16 и 24,00% соответственно. По мере увеличения оценки по КШОНН происходило увеличение доли пациентов, нуждавшихся в высокочастотной искусственной вентиляции легких, инфузии дофамина и адреналина. При анализе исходов госпитального этапа лечения наблюдалось достоверное увеличение доли умерших пациентов в подгруппах по мере роста оценки по КШОНН с 0,76 [0,02; 4,18]% в подгруппе 0–2 балла до 42,86 [21,82; 65,98]% в подгруппе 9–14 баллов. Кроме того, отмечен рост доли пациентов, у которых сформировались тяжелые внутрижелудочковые кровоизлияния, 0,00 [0,00; 2,78]% в подгруппе 0–2 балла и 19,05 [5,45; 41,91]% в подгруппе 9–14 баллов. Аналогичная закономерность наблюдалась в отношении частоты развития позднего неонатального сепсиса.</p></sec><sec><title>Заключение</title><p>Заключение. Исследуемая угрозометрическая шкала демонстрирует достоверное разделение пациентов по степени тяжести и исходам госпитального этапа лечения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose</title><p>Purpose. To study the patient’s characteristics and hospital outcomes in subgroups depending on the PNCAS scale score.</p></sec><sec><title>Material and methods</title><p>Material and methods. The cohort study included data from 604 trips of the transport team to newborns hospitalized in medical organizations of the Sverdlovsk region from August 1, 2017, to December 31, 2018. Median birth weight [IQR] 2515 [1600; 3275] grams, median gestational age [IQR] 36 [32; 38] weeks. The total sample was divided into subgroups depending on the assessment of the score, followed by a comparison of characteristics and outcomes in these subgroups.</p></sec><sec><title>Results</title><p>Results. There are significant differences in the structure of birth weight and gestational age, as the PNCAS score increases, there is a significant increase in the proportion of patients weighing less than 1000 grams and a decrease in the proportion of patients weighing 2500–3499 grams. The largest proportion of extremely premature newborns was observed in 6–8 points and 9–14 points subgroups, 30.16% and 24.00%, respectively. Assessment of the intensive care showed an increase in the proportion of patients requiring HFOV, dopamine and epinephrine infusion while increasing PNCAS score. Analysis of the outcomes showed a significant increase of mortality while increasing PNCAS score, 0.76% [0.02; 4.18] in the 0–2 points subgroup and 42.86% [21.82; 65.98] in the 9–14 points subgroup. There is also an increase in the proportion of patients who have formed severe IVH, 0.00% [0.00; 2.78] in the 0–2 points subgroup and 19.05% [5.45; 41.91] in the 9–14 points subgroup. A similar pattern is observed in the frequency of late onset sepsis.</p></sec><sec><title>Conclusion</title><p>Conclusion.
