Продуктом неферментативной деградации креатина (креатинфосфата) является креатинин, который накапливается в сыворотке крови, а его концентрация вероятнее всего коррелирует с объемом мышечной ткани пациента, при условии нормального функционирования почек.</p><sec><title>   Цель исследования</title><p>   Цель исследования. Оценка периоперационного уровня креатинина в сыворотке крови в качестве биологического маркера мышечной массы у пациентов с тяжелыми формами детского церебрального паралича (IV–V функциональный уровень по GMFCS) при ортопедических вмешательствах на тазобедренном суставе.</p></sec><sec><title>   Материалы и методы</title><p>   Материалы и методы. Проспективное клиническое обсервационное исследование включало 82 пациентов с тяжелыми формами церебрального паралича, спастическими вывихами (подвывихами) бедра, по поводу чего проводились реконструктивные или паллиативные вмешательства на тазобедренных суставах. Оценивали трофологический статус детей перед операцией, определяли уровень креатинина в сыворотке крови в интраоперационном периоде, в 1-й послеоперационный день и на 5-й день после операции.</p></sec><sec><title>   Результаты</title><p>   Результаты. Окружность средней трети плеча у 28 % пациентов, а также толщина кожно-жировой складки над трицепсом у 61 % детей находились ниже 10-го центиля, что расценивалось как недостаточное питание. Рассчитанная доля телесного жира, равная 10 % [8; 20] в комплексе с окружностью мышц средней трети плеча, позволяет предположить у 1/5 пациентов сочетание белково-энергетической недостаточности и дефицита мышечной массы. Уровни креатинина на всех этапах наблюдения соответствовали минимуму возрастной нормы или находились ниже этих значений, имели тенденцию к снижению и случайный характер согласования (W = 0,129).</p></sec><sec><title>   Заключение</title><p>   Заключение. Уровень креатинина в сыворотке крови коррелирует с таким показателем, как окружность мышц плеча у детей с тяжелыми формами церебрального паралича, выраженными двигательными расстройствами (IV–V функциональный уровень по GMFCS). Уровень креатинина в плазме крови может использоваться в качестве биологического маркера массы скелетной мускулатуры у пациентов с тяжелыми формами церебрального паралича.</p></sec></abstract><trans-abstract xml:lang="en"><p>   Energy processes in muscles can be reflected by the creatine phosphokinase system of ATP resynthesis from ADP and creatine phosphate. The product of non-enzymatic degradation of creatine (creatine phosphate) is creatinine, which accumulates in the blood serum, and its concentration correlates with the volume of the patient’s muscle tissue, subject to the normal functioning of the kidneys.</p><sec><title>   Purpose</title><p>   Purpose. To assess the perioperative serum creatinine level as a biological marker of muscle mass in patients with severe forms of cerebral palsy, IV–V level according to GMFCS, during orthopedic interventions on the hip joint.</p></sec><sec><title>   Material and methods</title><p>   Material and methods. A prospective clinical observational study included 82 patients with severe forms of cerebral palsy, spastic dislocations (subluxations) of the hips, for which reconstructive or palliative interventions were performed on the hip joints. The trophological status of children was assessed before surgery, the level of serum creatinine was determined in the intraoperative period, on the first postoperative day, and on the fifth day after surgery.</p></sec><sec><title>   Results</title><p>   Results. The circumference of the middle third of the shoulder in 28 % of patients, as well as the thickness of the skin-fat fold over the triceps in 61 % of children were below the 10th centile, which was regarded as malnutrition. The calculated proportion of body fat equal to 10 [8; 20] % in combination with the circumference of the muscles of the middle third of the shoulder suggest a combination of protein-energy deficiency and muscle mass deficiency in 1/5 of the patients. Creatinine indicators at all stages of observation corresponded to the minimum age norm or were below these values, tended to decrease and had a random agreement (W = 0,129).</p></sec><sec><title>   Conclusion</title><p>   Conclusion. The level of creatinine in the blood serum correlates with the «shoulder muscle circumference» parameter in children with severe forms of cerebral palsy, severe movement disorders of IV–V level according to GMFCS.
