Оценка специфического Т-клеточного и гуморального иммунитета против SARS-CoV-2 у детей, госпитализированных с респираторными инфекционными заболеваниями.Материалы и методы. Проведено обследование 121 ребенка в период с 2021 по 2024 г. Среди них 22% были младше 1 года, 33% — в возрасте 1–3 лет, 21% — в возрасте 4–6 лет, 24% — в возрасте 7–14 лет. У 51 ребенка острая респираторная инфекция сопутствовала течению коклюша. Диагностика острых респираторных вирусных инфекций основывалась на клинических данных и результатах исследования слизи из носоглотки методом ПЦР, в том числе и на SARS-CoV-2. По разработанному нами методу оценено наличие Т-клеток памяти к S-гликопротеину SARS-CoV-2 в крови пациентов. Кроме того, определена концентрация IgG- и IgМ-антител к S-гликопротеину SARS-CoV-2 в сыворотке крови.Результаты. IgG-антитела к S-гликопротеину SARS-CoV-2 в 2021 г. выявлены в крови у 55% детей, в 2022 г. — у 65%, в 2023 г. — у 82%, в 2024 г. — у 96%. Т-клетки памяти при их стимуляции рекомбинантным S-гликопротеином SARS-CoV-2 были обнаружены в 2021 г. в крови у 25% детей, в 2022 г. — у 70%, в 2023 и 2024 гг. — у 19–35%. У детей в возрасте до 3 лет, контакты которых особенно в 2021 г. были ограничены семьей, выявлена значительная (55%) частота серологических маркеров перенесенного недиагностированного COVID-19. В группах пациентов 3–10 лет серопозитивных было еще больше (95–100%). Т-клетки памяти у детей до 3 лет обнаруживались реже, чем после 3 лет.Заключение. Обследование лиц, переносящих различные респираторные заболевания, может уточнить представления о том, какая часть населения приобрела иммунитет против SARS-CoV-2, а также оценить роль Т-клеток памяти и антител к «предковым» и актуальным штаммам SARS-CoV-2 в ограничении заболеваемости.</p></abstract><trans-abstract xml:lang="en"><p>Objective. The aim was to evaluate specific T-cell and antibodies against SARS-CoV-2 in children hospitalized with respiratory infectious diseases.Materials and methods. Examination of 121 children was carried out in 2021–2024. Among them 22% of patients were under 1 year of age, 33% — 1–3 years, 21% 4–6 years, 24% 7–14 years. In 51 children acute respiratory infection was concomitant with the course of pertussis. Diagnosis of acute respiratory viral infections was based on clinical data and on PCR testing of nasopharyngeal mucus including PCR testing for SARS-CoV-2. Using a method we developed, memory T cells specific to the SARS-CoV-2 S-glycoprotein were assessed in patients' blood. IgG- and IgM-antibodies to SARS-CoV-2 S-glycoprotein in blood serum were also tested.Results. IgG antibodies to S-glycoprotein SARS-CoV-2 were detected in the blood of 55% of children in 2021, 65% in 2022, 82% in 2023, and 96% in 2024. T lymphocytes activated by stimulation with recombinant S-glycoprotein SARS-CoV-2 were detected in the blood of 25% of children in 2021, 70% in 2022 and 19–35% in 2023–2024. A significant (55%) frequency of serologic markers of transmitted undiagnosed COVID-19 was found in children under 3 years of age whose contacts, especially in 2021, were limited to family. In the groups of patients 3–14 years old, even more children were seropositive (95–100%). Memory T cells were found less frequently in children younger 3 years than after 3 years of age.Conclusion.
