В настоящее время затруднен дифференциальный диагноз COVID-19 с гастроинтестинальным синдромом и острых кишечных инфекций, что требует разработки алгоритма их диагностики, лечения и прогнозирования рисков развития осложнений.</p><sec><title>Цель исследования</title><p>Цель исследования. Оценить клинико-эпидемиологические особенности диарей различной этиологии в период пандемии COVID-19 у детей, госпитализированных с COVID-19 с гастроинтестинальными симптомами.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Обследовано 105 пациентов до 18 лет с COVID-19 и гастроинтестинальным синдромом. Обследование включало ежедневный осмотр, лабораторные исследования для оценки клинических особенностей течения. Диагностику острых кишечных инфекций проводили методами ПЦР и бактериологическим исследованием кала.</p></sec><sec><title>Результаты</title><p>Результаты. Из 105 госпитализированных пациентов с диагнозом COVID-19 с гастроинтестинальным синдромом у 57 (55%) обнаружена сопутствующая вирусная (41%) и бактериальная (13,3%) кишечная инфекция. В группе COVID-19 + вирусные инфекции выявлен высокий показатель максимальной кратности рвоты в сутки (p=0,04). При гастроинтестинальной форме COVID-19, а также сочетании COVID-19 с бактериальными кишечными инфекциями у 20,8% и 35,7% пациентов отмечается повышение уровня креатинина до 119 и 80 мкмоль/л, соответственно (р=0,01). В группе COVID-19 с бактериальными кишечными инфекциями у 57,1% отмечен риск развития острого повреждения почек. Вероятность развития начальной стадии острого повреждения почек выше в группе COVID-19 с гастроинтестинальным синдромом.</p></sec><sec><title>Заключение</title><p>Заключение. Полученные данные свидетельствуют о частой (30%) встречаемости гастроинтестинальных проявлений при COVID-19 у детей. Течение заболевания в группах детей с COVID-19 и гастроинтестинальным синдромом и COVID-19 c бактериальными кишечными инфекциями осложняется риском развития острого почечного повреждения, что требует дальнейшего исследования для раннего прогнозирования поражения почек и своевременного лечения.</p></sec></abstract><trans-abstract xml:lang="en"><p>Against the background of the COVID-19 pandemic, an increase in the total proportion of viral diarrhea in the etiological structure of acute intestinal infections in children has been recorded. Currently, the differential diagnosis of COVID-19 with gastrointestinal syndrome and acute intestinal infections is difficult, which requires the development of an algorithm for their diagnosis, treatment and prediction of the risks of complications.</p><sec><title>Objective</title><p>Objective. To evaluate the clinical and epidemiological features of diarrhea of  various etiologies during the COVID-19 pandemic in children hospitalized with COVID-19 with gastrointestinal symptoms.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. We examined 105 patients under 18 years old with COVID-19 and gastrointestinal syndrome. The examination included daily examination, laboratory tests to assess the clinical features of the course. Diagnosis of acute intestinal infections was carried out using PCR methods and bacteriological examination of feces. </p></sec><sec><title>Results</title><p>Results. Of 105 hospitalized patients diagnosed with COVID-19 with gastrointestinal syndrome, 57 (55%) had concomitant viral (41%) and bacterial (13.3%) intestinal infections. In the COVID-19 + viral infections group, a high rate of maximum frequency of vomiting per day was revealed (p=0.04). In the gastrointestinal form of COVID-19, as well as a combination of COVID-19 with bacterial intestinal infections, an increase in creatinine levels to 119 and 80 μmol / L was noted in 20.8% and 35.7% of patients, respectively (p=0.01). In the COVID-19 with bacterial intestinal infections group, 57.1% had a risk of developing acute kidney injury. The likelihood of developing the initial stage of acute kidney injury is higher in the COVID-19 with gastrointestinal syndrome group.</p></sec><sec><title>Conclusion</title><p> Conclusion. The obtained data indicate a frequent (30%) occurrence of gastrointestinal manifestations in COVID-19 in children.
