Одним из частых проявлений синдрома холестаза при синдроме Алажилля является интенсивный зуд, который существенно снижает качество жизни пациентов. Существующие стандартные подходы к противозудной терапии, в том числе урсодезоксихолевая кислота, секвестранты желчных кислот, рифампицин, анксиолитики и др., не обладают достаточной эффективностью. В настоящий момент активно исследуются и внедряются в терапию ингибиторы транспортера желчных кислот (IBAT), такие как мараликсибат, предотвращающий энтерогепатическую рециркуляцию желчных кислот и как следствие снижающий их концентрацию в крови, что ведет к уменьшению зуда. В представленном исследовании мы оценили эффективность мараликсибата у детей с синдромом холестаза и зудом при синдроме Алажилля.</p><sec><title>Материалы и методы</title><p> Материалы и методы. В исследование включены 12 детей с синдромом Алажилля (возраст 53,83±48,23 месяцев). У всех детей отмечался интенсивный кожный зуд и как минимум четырехкратное повышение уровня свободных желчных кислот в крови. Холестатический зуд оценивался с использованием шкалы IthRo (0–4 балла) родителями пациентов. </p></sec><sec><title>Результаты</title><p>Результаты. Один пациент не продолжил наблюдение после назначения препарата, 11 пациентов получали мараликсибат ≥12 недель, 7 — более 48 недель. К 12 неделе у 86% пациентов уровень желчных кислот на терапии снизился более чем на 50%, а к 48 неделе отмечено более чем трехкратное снижение концентрации желчных кислот. Интенсивность зуда уменьшилась на ≥2 балла к 4 неделе терапии у 9 (82%) пациентов. К 48 неделе интенсивность зуда не превышала 1,17 балла (95% CI 0,38–1,96) у 5 из 7 пациентов. У одного пациента не удалось достигнуть клинически значимого уменьшения зуда, но двукратное увеличение дозы мараликсибата на 36 неделе терапии оказало положительный эффект в виде снижения интенсивности зуда до 1 балла по шкале ItchRo к 48 неделе терапии.</p></sec><sec><title>Выводы</title><p>Выводы. Данное исследование демонстрирует высокую эффективность ингибиторов обратного транспорта желчных кислот в лечении холестатического зуда у пациентов с синдромом Алажилля.</p></sec></abstract><trans-abstract xml:lang="en"><p>Alagille syndrome is a rare hereditary disease characterized by cholestasis syndrome due to bile duct hypoplasia. One of the frequent manifestations of cholestasis syndrome in Alagille syndrome is severe pruritus, which significantly reduces the quality of life. Existing standard approaches to antipruritic therapy, including ursodeoxycholic acid, bile acid sequestrants, rifampicin, anxiolytics, etc., is not effective enough. Currently, bile acid transport inhibitors (IBAT), such as maralixibat, which prevents enterohepatic bile acids recirculation and, as a result, reduces it’s blood concentration, which leads to a decrease of pruritus, are actively studied and introduced into therapy.</p><sec><title>Methods</title><p>Methods: the study included 12 patients with Alagille syndrome (age 53.83±48.23 months). All children had severe pruritus and &gt; four-fold increase of serum bile acid. Cholestatic pruritus was assessed by patients’ parents using the IthRo scale (0–4 points).</p></sec><sec><title>Results</title><p>Results: One patient did not continue participation in the study after the drug prescription, 11 patients received maralixibat for ≥12 weeks, 7 for &gt; 48 weeks. By week 12,86%, had a decrease of blood serum bile acid &gt;50%, and by week 48 &gt;three-fold decrease in blood serum bile acid concentrations. Pruritus severity decreased by ≥2 points by week 4 of therapy in 9(82%) patients. By week 48, pruritus intensity did not exceed 1.17 points (95% CI 0.38–1.96) in 5 of 7 patients.
