В обзоре представлена информация об известных механизмах удлинения интервала QT у пациентов с полной атриовентрикулярной блокадой. Представлены подтверждения значимости изменений процессов реполяризации для развития жизнеугрожающих желудочковых тахиаритмий при полной атриовентрикулярной блокаде, в том числе у детей. Многообразие механизмов, влияющих на реполяризацию миокарда у пациентов с полной атриовентрикулярной блокадой, значительно утяжеляет течение и прогноз заболевания, затрудняет подходы к лечению пациентов, особенно в раннем возрасте. Определение предиктивных значений параметров реполяризации у детей крайне важно для профилактики развития аритмогенных синкопальных состояний и внезапной сердечной смерти.</p></abstract><trans-abstract xml:lang="en"><p>Sudden cardiac death is a serious problem that accounts for approximately 50% of deaths due to cardiovascular diseases. This review provides information about the known mechanisms that prolong the QT interval in patients with complete atrioventricular block. It also presents evidence of the significance of changes in repolarization processes for the development of life-threatening ventricular tachyarrhythmias, including in children. The variety of factors affecting myocardial repolarization in patients with complete atrioventricular block significantly worsens the course of the disease and prognosis and complicates approaches to the treatment of patients, especially in young children.
