Состояние здоровья и развитие детей, рожденных после экстракорпорального оплодотворения

Аннотация

Предмет обзора данных литературы: состояние здоровья и развитие детей, рожденных после экстракорпорального оплодотворения (ЭКО). Проведен анализ данных отечественных и зарубежных публикаций за период с 1995 до 2013 г. с преимущественным вниманием к крупномасштабным многоцентровым исследованиям. Результаты. Негативные перинатальные исходы (невынашивание, маловесность и их осложнения) обусловлены преимущественно «факторами со стороны родителей» — бесплодием, возрастом и сопутствующим комплексом метаболических, генетических и эпигенетических изменений, что отражается на качестве гамет и течении беременности, а также, вероятно, обусловливает повышенный (1,3—2) риск врожденных аномалий развития и потребность в их хирургической коррекции у детей после ЭКО в сравнении с детьми, родившимися в результате спонтанных беременностей. Единственный доказанный фактор субоптимальных перинатальных исходов, ассоциированный с методом ЭКО, — многоплодие. Данные о повышенной частоте соматической и другой патологии у детей после ЭКО противоречивы и не подтверждены крупными исследованиями; сравнение уровня генетических и эпигенетических нарушений затруднительно ввиду их редкой встречаемости. Открытым остается прогноз репродуктивного статуса и отсроченных заболеваний (онкологии, метаболических нарушений) у данной группы детей.
