Поражение сердца и коронарных артерий при синдроме Кавасаки
Аннотация
Синдром Кавасаки — острый системный васкулит неизвестной этиологии, которым болеют преимущественно дети первых 5 лет жизни. В настоящее время синдром Кавасаки признан ведущей причиной приобретенных органических заболеваний сердца у детей, которые могут приводить к ишемической болезни сердца, инфаркту миокарда и внезапной смерти у детей и лиц молодого возраста. Большинство осложнений связаны с сердечно-сосудистой системой, особенно с изменениями коронарных артерий. Основным методом исследования при синдроме Кавасаки является трансторакальная эхокардиография, которая, помимо оценки коронарных артерий, позволяет оценить систолическую и диастолическую функции правого и левого желудочков, изучить состояние клапанов сердца, динамику перикардиального выпота. В статье изложены современные данные об эпидемиологии, этиологии, патогенетических механизмах и патоморфологии синдрома Кавасаки. Рассмотрены возможные коронарные и некоронарные изменения, исходы и клинические проявления.
