Жизнеугрожающая бронхиальная астма у детей: от предикторов до прогноза

Аннотация

Угрожающее жизни обострение бронхиальной астмы может возникнуть при любой тяжести заболевания. Определение предикторов фатального состояния позволит выделить группы высокого риска развития астматического статуса. В настоящее время традиционная интенсивная терапия астматического статуса дополнилась инновационными подходами в респираторной поддержке, бронходилатационной терапии, базисном сопровождении пациентов, перенесших жизнеугрожающую астму. К предикторам жизнеугрожающей бронхиальной астмы относят риновирусную инфекцию, тяжелое обострение в анамнезе, грибковую сенсибилизацию и возраст пациентов. Данное состояние на 25% увеличивает риск развития повторных угрожающих жизни обострений, что требует особого подхода к терапии и наблюдению пациентов на амбулаторном этапе. Данный обзор обращает внимание на современные подходы в терапии таких пациентов с целью унификации тактики ведения детей с астматическим статусом в Российской Федерации.
